I think this is an innovative example of knowledge creation, sharing and application....
Academic Alternatives - a simple model which develops a reflective space for interdiciplinary peer-based learning. This is a grassroots student-designed initiative in Waterloo which has brought community members, undergraduate and graduate students into a participant-driven learning space.
LSPIRG Strat Planning - a set of non-hierarchal, student-led strategic planning process which help student activists reflect on their learning and use their experiences to collectively plan future activities
LSPIRG/CSL Certificate in Community Training Program - Certificate in Community Training Program: a program meant to train students, community members, social entrepreneurs, and activists to work effectively in the not for profit sector and apply creative problem solving techniques to their communities. This is a student-designed and led initiative that includes extensive partnerships with Community Service Learning and local NGOs
My example of an innovative university structure or policy that supports new forms of knowledge creation, sharing and application is….
I have been impressed by the power of community service learning as a way of grounding theory, and in a different way CBR as well.
Policies which allow students, staff and faculty to retain ownership of their patentable ideas seems to be a good way of expanding the role of the university into the local economic system.
Competitions in which students pitch their business ideas are effective as well, but the even more interesting pitch competitions are those which include social enterprise and non-profit pitches. The LaunchPad50k competition has recently incorporated these.

Academic Alternatives v1.5

The Academic Alternatives model for organizing a discussion group and focuses discussion on a group's responses to a single question.
